Cable Royalty Claim Forms

The Copyright Royalty Board offers online and printable forms for filing of claims to cable royalty fees. Online submission of cable claims is strongly encouraged.

Cable claims must be filed by 5 p.m. July 31, 2009.

single claim (on behalf of a single copyright owner of a work or works)

joint claim (on behalf of more than one copyright owner)

Printable Forms:

You may complete the form on the website and then print it off and submit it to the Board. Alternative versions of the printable forms will not be accepted. You must use the actual printable forms posted on the website.
