NLS has elected to provide standard equipment control forms in portable document format (PDF) via the Internet. These forms will no longer be available from NLS in print.
When you select the document of your choice, it will open in Acrobat Reader. Use your local printer to print the desired quantity. Two-sided documents will have to be processed twice.
Note: These forms are not meant to be completed online so they should not raise any accessibility concerns.
