Request Form Information

Galley File Information & Core Required Materials
The CIP Program requires that a single galley file in PDF format be attached to the CIP Request in PrePub Book Link.
Library of Congress staff create the CIP data based on the book provided galley, not the application. The application must be accompanied by a full text galley or at least the core required materials.
Is your forthcoming title a multivolume set or more than one physical volume?

Reprints
Is your forthcoming title a reprint edition? If so, please submit a new CIP Request in PrePub Book Link with the core required materials. There is a question in the CIP Request asking if the forthcoming book is a reprint of a previously published work. If the user clicks yes, the user is prompted to enter the title of the original edition. If you know the LCCN and/or ISBN of the original work, please include this in the Additional Comments in the CIP Request form.
Publisher-Provided Summaries
The CIP Program has always encouraged publishers to include summaries with their application for CIP data as they facilitate subject analysis and other aspects of the cataloging process. When submitting a CIP Request in PrePub Book Link, publishers are required to include an English-language summary of at least thirty-five characters.
Additionally, summaries should comply with the CIP Guidelines for Summaries.
Children’s and Young Adults’ Cataloging Program Summaries
One of the key elements of the Children’s and Young Adults’ Cataloging Program (CYAC) is the providing of a brief, non-critical, one-sentence annotation that describes the content of the work being cataloged without making any judgmental statements.
